Absentee Ballot Application

You may need to vote via absentee ballot if you will be unable to vote in person on an election day. If so, you need to request that a ballot be sent to you. Print this form, fill it out, and mail it to your county's Election Office. Be sure to read the section on absentee ballots on the VotesPA page before you assume you can vote via absentee ballot. If, for instance, you have a disability and your polling place is not accessible, you are entitled to vote via an Alternative Ballot. Or if you need to vote by Absentee Ballot and need someone to deliver your completed ballot, you need to fill out an additional form.
